There have been changes made to Freshman Orientation and to Packet/Schedule pick-up. See the updated newsletter for the new schedules.

The deadline for ordering a DVD for the Varsity Awards Night is June 30. It includes the production from the Varsity Awards Nights as well as the individual videos the teams submitted.

You must use this physical form for sports. The actual physical must be after May 1, 2010 for 10-11 fall, winter and spring sports.

All athletes must have health insurance. If you do not have health insurance through your family, you must buy school insurance before trying out. Do not mail in the form to the insurance company until we have made a copy of the check and form for our records. If you do it online, please print out the confirmation sheet and attach it to your physical form. Attention all football players: this year all levels (freshman, JV and Varsity) must have one of the varsity football plans.

All drivers (parent, coach and student) must fill out this form and give it to your coach if you are going to drive to or from games, team functions, etc. this year.

Every student who is driving with another student and every student who is driving another student must fill out this form. The driver must also fill out the Driver Form.

The Cougar Store will be open at the Parent Meeting on Dec. 1 and occasional basketball games. You may also order at www.ohsathleticboosters.org (PayPal) or email to cougarstore2@gmail.com
